Appointments
To schedule an appointment, please go to counselingcenter.dbu.edu and fill out the Sign-Up form or come to the Counseling Center on the 1st floor of the Collins Learning Center to complete the form. Once you have completed and submitted your forms, a Counseling Center staff member will contact you to schedule your first appointment. Since our appointment slots fill up quickly, you could potentially be placed on a waitlist. We want to help you get scheduled with a counselor as soon as possible, so please provide as much availability as you can and be sure to answer phone calls or emails from the Counseling Center. Counseling appointments take place 8:00 a.m. – 8:00 p.m., Monday through Friday (call to ask about Saturday availability).

Counseling Records
The confidentiality of counseling matters will be maintained in accordance with the Health Insurance Portability Act of 1996 (HIPPA) and the Family Educational Rights and Privacy Act (FERPA).
Accessing Your Mental Health Records
You have the right to access your therapy records.
To formally request your records, please submit a written request via email to counselingcenter@dbu.edu. Once your request is received, we will contact you to verify its authenticity and coordinate any required authorization forms needed to release records to you or to outside parties. Please note that a reasonable fee may apply for copying and postage if paper records are requested. Additionally, depending on applicable record-retention laws, your records may not be available if a significant amount of time has passed since your file was archived.
How to File a Complaint
You have the right to file a complaint if you believe you have experienced unprofessional or unethical conduct. Below are the appropriate resources for submitting a formal complaint in the state of Texas.

To file a complaint, visit the BHEC Contact Page and select “Complaints and Enforcement” under the Professional Counseling section. Follow the instructions provided to download and submit the required form.
Consumer Protection Division

To file a consumer-related complaint, please visit the Attorney General of Texas Consumer Protection Division website and follow the instructions provided for submitting a complaint.
How to Submit Concerns about your Mental Health Care to the University
You have the right to submit a formal complaint or concern regarding your experience with your counselor and/or the DBU Counseling Center. You may contact the director at amyst@dbu.edu; you may contact your counselor’s clinical supervisor as stated in your signed informed consent; or you can contact the Vice President of Student Affairs.
